Sign Up for the 2-Mile-Fun Walk in Playa Vista on May 11th

I am voluntarily participating in the 2-Mile Fun Walk at 9:00 AM. I further acknowledge and agree the following:

Assumption of Risk: I understand that participating in physical activities such as urban walking involves certain risks, including but not limited to, accidents, injuries, and property damage. I voluntarily assume all risks to myself, and any pet or child I bring, associated with my participation and understand the walk leaders are volunteers who have no duty with respect to my safety nor any medical training or responsibility.

Physical Condition: I hereby certify that I am in sufficiently good health and physically capable for participating in this event. I have consulted with a medical professional if I have any doubts about my ability to safely participate.

Release and Waiver: In consideration for being permitted to participate in the volunteer weekly led walks and social activities, I hereby release and waive any and all claims, demands, actions, liabilities or causes of action against Nordic Body, its organizers, volunteers, sponsors, and any other associated individuals or entities, from any and all liability for any injuries, illness, damages, or losses that I (and/or my pet and/or minor) may sustain as a result of my participation in the walk and all social activities before or after the walk.

Photography and Publicity: I grant permission to Nordic Body to use any photographs, videos, or other media taken during the walk for promotional, marketing, and informational purposes.

Emergency Medical Treatment: In the event of an emergency, I release Nordic Body and its volunteers, representatives or other associated individuals and entities from any and all liability associated with being a Good Samaritan or otherwise calling for medical treatment to help me, including but not limited to, providing basic first aid, or other assistance in the event of a fall or other injury, calling an ambulance transportation etc. if necessary. I understand and agree that all costs associated with receiving medical care are my responsibility.
